Commitment to clients

• All treatments provided by Equilibrium – Holistic Treatments For Women are scheduled for a specific starting time and length.

• Clients who arrive late for an appointment or who wish to finish their session early must pay in full and cannot expect any missed time to be made up outside of their allocated time slot.

• Before the initial treatment a consultation form must be completed by the client. If any of the information provided on the consultation form changes, this needs to be communicated to the therapist before any follow on treatments are given.

• Treatments will only be given if there are no contra indications, which could make the treatments unsafe to have.

• Some conditions require GP / Consultant permission before treatments can be given.

• Depending on conditions present, in some cases, an Indemnity Form can be signed to ensure that treatments can be given.

• Clients presenting with symptoms of acute infection or illness on the day of their booking will be unable to receive a treatment, both for their own protection and that of the therapist. Examples include a heavy cold, flu, stomach upset, high temperature and the sudden onset of a severe headache.

• If you have a disability, please discuss what kind of support you require. We will do our best to offer the support you need.

 

Holistic treatments should not be used in place of conventional medical care. If you are concerned or not sure, either before booking an appointment or after you have received a treatment, always consult your GP or a healthcare professional for advice and help.

• Any treatments you receive are received on a voluntary basis by you and clients are responsible for their own healthcare needs before and after the treatments.

• Holistic therapists at Equilibrium – Holistic Treatments For Women are trained in their respective therapies but have no general medical training. Opinions expressed by the therapist are opinions only and can never be considered medical advice.

Cancellations

​

• A booking is confirmed once Equilibrium – Holistic Treatments For Women and the client have both agreed a date and time for a treatment.

• Cancellations can be made by email, telephone or SMS text.

• If a cancellation is made more than 24 hours before a treatment, the client will be able to re-arrange the appointment without any cost.

 

If a cancellation is made within 24 hours of the treatment, the treatment charge will apply. This will as well apply to a booking made on the basis that it will be paid for using a Gift Vouchers,

• Equilibrium – Holistic Treatments For Women has the right to cancel a treatment without notice if the room or environment is unsuitable for the treatment to be provided, or if the therapist is taken ill.

• Equilibrium – Holistic Treatments For Women has the right to cancel a treatment without notice if the client is intoxicated. Payment for the treatment will still be due.

• Treatments that have to be cancelled will be re-scheduled at the earliest possible time.

Vouchers

​

• Gift Vouchers can be used as payment towards any available, advertised treatments.

• Vouchers are only valid for one use and within their expiry date.

• If the whole amount of the voucher is not used, the remainder can be used as a part payment towards a further treatment within the next 6 months.

• There are no refunds for vouchers issued.

Payments

• Payment is accepted in cash, cheque or bank transfer.

Privacy

​

• Client treatment details are confidential and any notes/consultation forms will be stored securely.

• All clients must complete a consultation form prior to receiving the 1st treatment and inform the therapist, before any further treatments, should their health condition have changed in any way.

• By signing the consultation form, clients give permission to Equilibrium – Holistic Treatments For Women to hold records and data about them.

• Personal information will not be shared with any third parties.

• Clients under the age of 18 must be accompanied by a parent/guardian.

• The parent/guardian must give permission for the treatment to be performed and must co-sign the client’s consultation form..

• If the client is younger than 14, the parent/guardian must be present in the room throughout the treatment.

Inappropriate Behaviour

​

• Equilibrium – Holistic Treatments For Women has the right to terminate a treatment immediately if the client demonstrates inappropriate behaviour of any kind (verbally and/or physically). Payment made will not be refundable.

Complaints Procedure

​

• If a client wishes to make a complaint about Equilibrium – Holistic Treatments For Women, this must be emailed to equilibrium-holistictreatmentsforwomen@hotmail.com

• Complaints must include the date and location of the incident, the full name of the complainant, details of the complaint and a desired outcome following the complaint.

• All complaints will be taken very seriously and a response will be provided within 28 days.

Liability & Insurance

The Client acknowledges that the Massage Therapist is a sole trader and that any liability as to the provision of a treatment is between an Individual and the Massage Therapist.

Law

This Agreement shall be subject to English Law and the sole jurisdiction of the English Courts.
